• I understand and agree that I am fully responsible of my well-being during and after my coaching sessions, and
subsequently, including my choices and decisions.
• I understand that coaching is not a substitute for professional medical advice or treatment.
• I understand that all comments and ideas offered by my Coach are solely for the purpose of aiding me in achieving my goals. I have the ability to give my informed consent and hereby give such consent to my Coach to assist me in achieving such goals.
• I understand I have voluntarily elected to undergo this Essential Oils Blend after the nature and purpose of this use has been explained to me, along with the risk and hazards involved.
• I understand it is impossible to list every potential risk and complication, I have been informed of possible benefits. risks, and complications. I also recognize there are no guaranteed results and that independent results are dependent upon age, physical conditions, and lifestyle and that there is the possibility I may require further
consultations to obtain the expected results at an additional charge.
• I have also, to the best of my knowledge, given an accurate account of my medical history. including all known
allergies or prescription drugs or products I am currently ingesting or using impeccably.
• I understand my Coach will protect my information as confidential unless I state otherwise in writing. If my Coach is ordered bye court to provide information or testify, she will do so in the extent the law requires.
• I have read and fully understand this agreement and all information detailed above.
• Dianne’s Essential Oils Coaching LLC, specifically disclaim all responsibility for any liability, loss or risk. personal or otherwise, which in incurred as a consequence, directly or indirectly, for the use and application of any materials used in coaching. I hereby release, waive. acquaint and forever discharge my Coach, any agents. successors, assigns, personal representatives or heirs and employees from every claim, suit action, demand or right to compensation for damages I may claim to have or that I may have arising out of acts or omissions by myself or by my Coach as a result of the advice given by my Coach or otherwise resulting from the coaching relationship contemplated by this agreement I further declare and represent that no promise, inducement or agreement not expressed in this agreement has been made to me In sign this agreement This agreement shall bind my heirs, executors, personal representatives, successors, assigns. and agents.
